Scott Tracy, PHD
Dr. Scott Tracy, is a professor of counseling in the Pennsylvania State System of Higher Education where he teaches graduate courses in clinical mental health, school and addiction counseling. Dr Tracy is CEO of Tracy Counseling Center in Uniontown and owner of Wellness Recovery, a state licensed addiction inpatient treatment facility. Dr. Tracy is a licensed counselor in the state of Pennsylvania, an approved clinical supervisor and certified k-12 school counselor. Dr. Tracy earned his doctoral degree in counseling from Duquesne University in Pittsburgh and holds a master’s degree in counseling as well as an undergraduate degree in science education from Cal U. Dr. Tracy’s specific research interests are in disaster mental health that was fueled by his early career work a paramedic. Dr. Tracy has been in clinical practice for over 20 years.
Dr. Tracy is a generalist, meaning he treats a wide variety of issues. Dr. Tracy has specialized training in adolescent psychology and in the treatment of PTSD.
Counseling is offered in a tranquil, private environment. Dr. Tracy coordinates care with local primary care physicians and has affiliation agreements with local psychiatrists for clients interested in medication management.
I provide evidence based psychotherapy for individuals of all ages, couples, and families. My goal is to help individuals, couples and families to effectively address and resolve their problems. I utilize established treatment methods to ensure my clients receive the best care possible, which equals the best outcome possible. Braelyn Tracy, PhD
Braelyn Tracy is a nationally certified school psychologist at Tracy Counseling Center. She recently completed her Doctor of Philosophy in school psychology with dual licensure at Duquesne. Braelyn’s doctoral research surrounds the assessment and treatment of childhood concussions by practicing school psychologists. Dr. Tracy has additional credentials in Autism assessment and childhood intellectual and emotional disorders. Braelyn holds an undergraduate degree in Psychology with a minor in biology from Queens University of Charlotte. She graduated from Queens University of Charlotte with a Bachelor of Science in 2017 and then went on to earn a Master of Science degree in Child Psychology from Duquesne University in 2018.
While completing her graduate degree at Duquesne, she worked at her family's private psychological practice, where she conducted intake interviews, administered psychological assessments, developed comprehensive treatment plans, and provided individual counseling to children with a wide range of behavioral health issues. Additionally, she worked as a counselor at a 19-bed detox and short-term rehab addiction treatment facility, where she provided complex counseling and treatment services to adults experiencing problems related to addiction, mental health, and family crises, utilizing various therapeutic methods and techniques. Braelyn has previous experience of conducting assessments and interventions using a multisystemic and multitiered service delivery approach and working with people who have developmental disorders, behavioral disorders, and mood disorders. She regularly collaborates with schools and other treatment providers to ensure comprehensive care.
While at Duquesne, she participated in research related to childhood chronic illnesses, neuropsychological assessments and interventions, traumatic brain injuries, and autism. Braelyn is presently completing her dissertation, which intends to raise awareness about the identification, assessment, and intervention of students with concussions in schools and the importance of a multisystemic and multitiered approach to service delivery. She has gained skills in conducting psychological assessments utilizing such instruments as the Autism Diagnosis Observation Schedule (ADOS), as well as other cognitive, developmental, and pre-academic measures. In all aspects of her work, Braelyn considers an individual’s developmental perspective on both processes of therapeutic change and Adlerian concepts of childhood setbacks. She utilizes a holistic and individualized approach to treatment to address each client's unique needs. These ideas bridge the gap between clinical practice and empirical research. She places a premium on developing positive relationships as the cornerstone of treatment and employs evidence-based treatments such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and behavior management techniques.

Mark Santore
Mark Santore is a counselor at Tracy Counseling Center. Mark is a dedicated and compassionate therapist who recently earned a Master’s degree in Clinical Mental Health Counseling from St. Bonaventure University. Mark holds an undergraduate degree in Psychology from West Virginia University. Currently working towards licensure, Mark is passionate about helping individuals navigate life’s challenges, improve mental health, and achieve personal growth. With a focus on anxiety, depression, trauma and relationships, he uses a client-centered approach, ensuring that therapy is tailored to the unique needs of each person. Mark is committed to ongoing professional development and provides a safe, supportive space for healing and self-discovery.

Tyra Chiaravalle-Holmes
Tyra Chiaravalle is a counselor at Tracy Counseling Center. Tyra is currently pursuing her Master’s degree in Clinical Mental Health Counseling at Pennsylvania West University with the expectation to graduate in Fall 2025. Her post-baccalaureate specializes in Substance Use Disorders. Tyra graduated Summa Cum Laude from Pennsylvania West University with a Bachelors of Science in Psychology. Tyra’s counseling philosophy focuses on advocating for clients' rights and needs both within and outside the therapeutic space. Tyra has over three years of expertise providing direct clinical treatment while working at Wellness Recovery, our inpatient detox and rehabilitation center. With a strong foundation in evidence-based practices, Tyra gained experience working with clients facing issues such as anxiety, depression, trauma, and substance abuse difficulties. She believes in creating a safe and supportive space where clients can feel heard, understood, and empowered to make meaningful changes in their lives.

Gregory Beaver
Greg Beaver is a licensed counselor at Tracy Counseling Center. Greg is a licensed therapist with over 15 years of experience helping individuals and couples strengthen their relationships and overcome personal challenges. His passion is working with couples, helping them navigate communication issues, conflicts, intimacy concerns, and life transitions to build deeper, more fulfilling connections. Greg uses a variety of evidence-based approaches, including Emotionally Focused Therapy (EFT) and the Gottman Method, to guide couples toward better understanding and connection. Whether you're facing recurring conflicts, dealing with infidelity, or simply seeking to improve your relationship dynamics, Greg’s here to support you in creating the healthy, loving relationship you desire. Greg aims to provide a safe, nonjudgmental space where both partners can feel heard, respected, and understood as you work together toward lasting change.

Nicole Doss
At the beginning of 2024, Nicole joined our team as an administrative assistant. She is primarily responsible for providing administrative support to the staff, scheduling appointments, and supporting daily administrative duties and operational functions by collaborating with all staff members, including clinicians, interns, and the billing coordinator. She is also responsible for all aspects of internal and external communications and serves as the primary point of contact for external offices.
Nicole is a dedicated mental health professional with over 14 years of experience serving the community through her work with the mental health association of Fayette County. Known for her empathy, clinical skills, and dedication to public service, Nicole is committed to reducing stigma and improving mental health access within her community. Her collaborative approach with Fayette County Behavioral Health Administration and other professionals in the field enhances the overall well-being of those we serve, making her an invaluable asset to our team.
Her passion for mental health advocacy and her in-depth understanding of county resources allow her to navigate complex systems, ensuring our patients receive the best possible care. She has an optimistic, hardworking attitude and a genuine desire to be of service to others, making her an asset in any workplace. She has an extensive background in community outreach and will likely be the friendly voice you hear when you call the office!
